Honoree: Founded on October 23, 1946, by the late Reverend Jeremiah Crawford, Union Grove Missionary Baptist Church (UGMBC) has served thousands of New Yorkers of all backgrounds, age and race. From a membership of 13 at its original location at 221 East 98th Street, UGMBC has grown into one of the most respected members of the community in the Bronx, where they currently operate at 1488 Hoe Avenue, and the City. Currently under the leadership of Reverend Frederick Crawford, the third generation of the Crawford family to pastor at UGMBC, Union Grove continues to carry out many of the community service programming created under Rev. Jeremiah Crawford. Leaned upon by community leaders and elected officials alike, UGMBC has served as a critical partner to enhance the quality of life of New Yorkers. UGMBC’s congregants have spearheaded numerous volunteer opportunities to serve the community beyond the walls of the parish. For those less fortunate, UGMBC runs food distribution programs and organizes an annual winter coat program that collects warm clothing to distribute to members of the community. During the warmer months, UGMBC organizes outdoor community events to give neighborhood youth a safe place to enjoy and play. (Salamanca)

Urban Health Plan Boulevard (Bronx)

Honoree: Urban Health Plan, Inc. (UHP), opened in 1974, provides affordable primary and specialty health care services to the Hunts Point, Mott Haven and Morrisania communities of the Bronx, Central Harlem and Corona, Queens. Facilities include the original site, nine satellites, a mental health facility, twelve school health programs, three part-time facilities, and six administrative/program sites. UHP began as a group practice with the mission of providing basic and specialty medical care, comparable to those found in local hospital outpatient clinics, to the community’s predominantly Hispanic and poor residents. It originated with Dr. Richard Izquierdo, a pediatrician and family practitioner who opened the San Juan Health Center in 1967. Seven years later, UHP, a private, non-profit corporation was established to broaden the scope of the work started by the San Juan Health Center. Over the years, UHP has allied with several community-based organizations to provide health care services at alternate delivery sites. These sites consist of local schools, an adult day treatment program, a Boys and Girls Club and homeless shelters. UHP has been actively involved with the U.S. Department of Health and Human Services, Bureau of Primary Health Care Health Care Disparities Collaborative, and has received national recognition for its performance improvement work. Its work with the asthmatic population in the South Bronx has contributed to the reduction in pediatric hospitalizations related to asthma as reported by the N.Y.C.
